Important Information About Use

After purchase (payment), the checkup program cannot be changed, and the gift voucher cannot be returned or refunded.

Please transfer payment to the designated account within seven days of applying (within 10 days for phone applications). The transfer receipt will serve as your receipt. If payment is not received by the deadline, the application will be treated as cancelled.

As a general rule, the voucher will be mailed to the purchaser.

The person who will undergo the checkup must make the appointment. As the preferred date may not be available, please book well in advance.

When making the appointment, please state that you will be using this gift voucher.

Any fees for optional examinations requested at the time of booking are the responsibility of the user.

If a physician determines that a biopsy is needed during upper gastrointestinal endoscopy and a tissue sample is taken from the stomach lining, a separate charge will apply under insured medical care. This cost is the responsibility of the user.

On the day of the examination, please bring the gift voucher and present it at reception. The voucher cannot be used if it is not presented.

The gift voucher cannot be used together with other discount or benefit vouchers.

We cannot accept responsibility for theft, loss, destruction, or similar loss of the gift voucher.

The gift voucher is valid for two years from the issue date shown on the reverse. It cannot be used after the expiry date.
